Poké Balls are used for capturing and containing wild Pokémon. They may not be used to capture a Trainer's Pokémon.
Poké Balls may be used outside of, or during, a battle with a wild Pokémon. When you are not engaged with a Pokémon, simply right click the mouse to throw the Poké Ball in hand. This technique requires that you aim at the Pokémon you are intending to capture by placing the mouse cursor over the Pokémon. Using this technique there is a small chance that the Ball you throw may break on contact with the Pokémon. If this happens you can collect the broken pieces and reconstruct the Ball in a crafting table.
If you are engaged in a Pokémon battle, during your turn, click on bag. Any Poké Balls in your Minecraft inventory will be under the Poké Balls tab. This method requires no aim as any Poké Ball you select will be automatically thrown at the wild pokémon.
